Privacy Policy and HIPAA
Thank you for choosing to use my services. Protecting your privacy is important at Simply Explained Health Advocacy. This privacy policy has been created to disclose to you why your personal information is collected and used, how long it is kept, how it is protected, and who has access to it.
Why It Is Collected and How It is Used
It is collected and used to fulfill the purposes for which it will be specifically used among the services listed and to communicate with you and/or your healthcare provider(s) on your behalf, as your professional advocate. Other uses include (but not limited to) complying with laws, assisting with public health and safety issues, for example, as in a pandemic.
How Long It Is Kept
Personal information will be retained only as necessary to fulfill the purposes of the services you require. If services are needed beyond one year, you will be notified about renewing your written consent to disclose your personal information before the end of that year. Without written consent, the advocacy services cannot be continued for you, or a loved one until this is renewed.
How It Is Protected
Your information will be protected in many ways: By reasonable security safeguards (locked area) and against loss, theft, copying or unauthorized access. In addition, security-enhanced digital cloud services, digital security monitoring (Norton), Microsoft 365 with its security features, and encrypted email are in place. In addition, a HIPAA-compliant software client management system is in place.
Who Has Access to the Information
In addition to the contact and other personal information you provide, your information will only be shared in the following ways: if required by law, for public safety reasons, to communicate with you and/or your healthcare providers, to assist with coordinating your care or to operate my business. Your information will not be shared for any marketing purposes without your written consent.
Changes to Privacy Policy
If any significant changes are made, the changes will be highlighted and posted.
